Pain in lower back is  very common in India. Approximately one-third of Indian population suffers from low back ache at least once in life time. It can occur due to injury or due to improper posture. Acute Low back pain starts abruptly and patient has to take rest and correct posture. If this pain persists for more than 3 months, then it is called chronic low back pain. Chronic pain is always difficult to treat and requires specialized care and interventional spine procedures (Spine injections) by pain specialist doctor with postural precautions and medicine compliance by patients.

Symptoms:

  • Dull-aching pain in lower back
  • Stiffness in back with or without muscle spasm
  • Sciatica (Pain travelling from lower back to buttocks, leg, and foot)
  • Tingling sensation, burning pain, sharp shooting pain or stabbing pain in lower back or legs (due to nerve compression)
  • Numbness or heaviness in leg or foot
  • Weakness of toes or foot (Foot drop) - Due to Radiculopathy
  • Pain that changes with posture i.e increase in back or hip pain with standing or lying down position.

Identify correct Diagnosis:
Every back pain is due to some reason which has to be found out and corrected completely. Only one-third of back pain is due to disc prolapse or disc herniation. We should not label every back pain to be caused by disc prolapse.

  • Mechanical Low back pain (Most common)
  • Lumbo-sacral strain
  • Disc Prolapse / Disc Herniation
  • Facet joint subluxation/ Facet joint arthropathy
  • Spondylitis
  • Spondylolysis
  • Spondylolisthesis
  • Lumbar canal stenosis
  • Tuberculosis spine (Pott's spine)
  • Spinal tumours
  • Sacroiliatis / Sacroiliac joint strain

Treatment:
Treatment is multidisciplinary and requires step-wise approach.

  • Medicines
  • Physiotherapy
  • Posture correction / Activity modification
  • Spinal manipulation
  • Spine injection procedures
  • Surgery  (Should be avoided as much as possible; Done in cases of sudden onset, progressive weakness of toes, foot with numbness, impaired control of bowel or bladder)
